Understanding Clozapine Side Effects: A Psychiatrist’s Insight Dr Babor Aganren, December 26, 2023December 24, 2023 Psychiatric medications can be complex and as a Psychiatrist, I do my best to simplify things. In this article, my aim is to explore clozapine, a powerful antipsychotic with its fair share of benefits and, like any medication, potential side effects. Clozapine’s Therapeutic Potential: Clozapine is renowned for its effectiveness in managing severe mental health conditions, particularly treatment-resistant schizophrenia. When prescribed judiciously, it can bring relief to individuals struggling with persistent symptoms that haven’t responded to other medications. Clozapine Forms Clozapine comes in three main forms: Tablets, Oral suspension and Orodispersible forms. In some few occasions, initial short course injection of Clozapine may be prescribed when there is evidence that an individual would likely continue oral Clozapine when more well in their mental health. Indications and Dose of Clozapine Clozapine is indicated for Schizophrenia in patients unresponsive to, or intolerant of, conventional antipsychotic drugs. It is also indicated for Psychosis in Parkinson’s disease. The BNF suggests the below Dose for Clozapine in Treatment resistant Schizophrenia “Adult 18–59 years 12.5 mg 1–2 times a day for day 1, then 25–50 mg for day 2, then increased, if tolerated, in steps of 25–50 mg daily, dose to be increased gradually over 14–21 days, increased to up to 300 mg daily in divided doses, larger dose to be taken at night, up to 200 mg daily may be taken as a single dose at bedtime; increased in steps of 50–100 mg 1–2 times a week if required, it is preferable to increase once a week; usual dose 200–450 mg daily, max. 900 mg per day, if restarting after interval of more than 48 hours, 12.5 mg once or twice on first day (but may be feasible to increase more quickly than on initiation)—extreme caution if previous respiratory or cardiac arrest with initial dosing. Adult 60 years and over 12.5 mg once daily for day 1, then increased to 25–37.5 mg for day 2, then increased, if tolerated, in steps of up to 25 mg daily, dose to be increased gradually over 14–21 days, increased to up to 300 mg daily in divided doses, larger dose at to be taken night, up to 200 mg daily may be taken as a single dose at bedtime; increased in steps of 50–100 mg 1–2 times a week if required, it is preferable to increase once a week; usual dose 200–450 mg daily, max. 900 mg per day, if restarting after interval of more than 48 hours, 12.5 mg once or twice on first day (but may be feasible to increase more quickly than on initiation)—extreme caution if previous respiratory or cardiac arrest with initial dosing”. Contra-indications For clozapine There are some conditions which would suggest that another medication, not Clozapine, might need to be considered. The BNF lists Contraindications to clozapine as Alcoholic and toxic psychoses Bone-marrow disorders Coma Drug intoxication History of agranulocytosis History of circulatory collapse History of neutropenia Paralytic ileus Severe cardiac disorders (e.g. myocarditis) Severe CNS depression and Uncontrolled epilepsy Clozapine Side Effects: Just like any prescription drug, clozapine comes with a list of potential side effects. It’s crucial to note that not everyone will experience these, and the severity can vary from person to person. This article explores lots more potential side effects. Here however, we discuss some commonly discussed clozapine side effects and how to manage. 1. Sedation and Drowsiness: Patients initiating clozapine therapy may notice increased sleepiness or drowsiness. This effect tends to be more pronounced during the initial stages of treatment but often diminishes over time. It’s advisable to exercise caution when engaging in activities that require alertness until you understand how your body responds to the medication. 2. Weight Gain: Weight gain is a well-documented side effect of clozapine. This can be attributed to changes in metabolism and increased appetite. Maintaining a healthy diet and incorporating regular physical activity can help mitigate this side effect. 3. Salivation and Drooling: Some individuals on clozapine may experience increased salivation or drooling, especially during sleep. This side effect is generally harmless but can be bothersome. It is worthwhile to note that constipation may lead to/ be a sign of more serious gastrointestinal issues like intestinal obstruction which requires emergency treatment. Constipation on clozapine should be taken seriously. 5. Agranulocytosis: One of the more serious but rare side effects of clozapine is agranulocytosis, a condition characterized by a significant decrease in white blood cell count. White blood cells are an important line of defense against invaders like bacterial and viral infection. Regular blood monitoring is essential to detect this potential complication early. Patients on clozapine typically undergo routine blood tests to ensure their safety. 6. Cardiovascular Effects Clozapine can have various effects on the heart and vessels. It can lead to the blood pressure going low (Hypotension), Blood pressure being High (Hypertension) or the heart beating fast (Tachycardia). Because of these issues, Blood pressure and Heart rates are monitored closely when people are starting on Clozapine and rate of increasing doses adjusted accordingly. Persistent Tachycardia can be managed with medications like Propranolol. With Hypotension, patients may be advised to stand up slowly. These issues tend to occur at the beginning periods of starting on Clozapine and could settle in time. Understanding clozapine side effects is vital for individuals considering or currently using this medication. While some side effects are more common, others are rare but potentially severe.
